The Minnesota Aging Pathways is the Minnesota Board on Aging’s free statewide information and assistance service. Minnesota Aging Pathways helps people and their families find local resources and make decisions about long-term services and support.

LeadingAge Minnesota is an association of organizations serving Minnesota seniors. If you go to their website, you can find many resources that can help you assess your needs and help you locate care and service options in your community.

ElderCircle

ElderCircle is a nonprofit agency that provides and coordinates a wide range of services for seniors in the Itasca County area. They also serve as a clearinghouse for information that is pertinent to seniors.

Chisago County Health and Human Services Director

Chisago County Health and Human Services offers a combination of Public Health and Social Services programs that support the citizens of Chisago County with their health, financial and social services needs.
